The 2022 iodinated contrast shortage was a supply shock. Getting through it was a <\/span>decision<\/span><\/b>.<\/span>\u00a0<\/span><\/p>\n

In the spring of 2022, radiology departments around the world opened their cabinets and found a problem they could not scan their way out of. A COVID-19 shutdown at a GE Healthcare plant in Shanghai, the site that produces the widely used iodinated contrast agent iohexol, cut global supply almost overnight. By mid-May, the manufacturer expected an eighty percent reduction in supply through the end of June.<\/span>\u00b9<\/span> Iodinated contrast is not a convenience. It is what makes a CT scan show a pulmonary embolism, a bleed, a tumour. When it runs short, the question is no longer what to image. It is who to image, and with what.<\/span>\u00a0<\/span><\/p>\n

What happened next is the part worth remembering. The shortage was the same for everyone. The outcomes were not.<\/span>\u00a0<\/span><\/p>\n

Some departments cancelled and deferred. Others kept moving, because they could make decisions quickly. They postponed routine treatment-monitoring scans for stable patients to protect contrast for emergencies. They substituted non-contrast CT, MRI or ultrasound where it was clinically safe. Pharmacies repackaged large single-use vials into smaller doses to stretch every millilitre. And the institutions that had signed contracts with more than one supplier simply drew on another source. One academic health system credited exactly that, its enterprise-wide contracts across different vendors, with softening the blow.<\/span>\u00b9<\/span>\u00a0<\/span><\/p>\n

Look closely at that list and you will notice something. Every one of those responses is a decision, not a product. Substitute. Reallocate. Reprioritize. Switch supplier. The contrast shortage was a supply shock, but getting through it was a decision problem. The departments that came through best were the ones that could see their exposure early and act on it fast, across every site at once.<\/span>\u00a0<\/span><\/p>\n

That is precisely the gap most radiology groups still carry into the next shortage. The data to make those calls usually exists. It sits in the inventory system, the ordering history, the supplier emails, the exam schedule. What is missing is the ability to turn it into the right move before the shelf is empty.
